Book excerpt: EMS providers learn the skills that allow them to give state-of-the-art care at the scene. Unfortunately, how to document that care so that it is not called into question two days or even two years down the road is often not taught in-depth. In order to help EMTs with this important subject, we assembled this water resistant, pocket-sized guide for providers at all levels, examining many of the common calls EMTs receive and following a standard protocol for the preparation of EMS run reports.
